Referring Physicians to Walsh Urology.Dear Referring Physician,

I welcome the privilege of working with you on the evaluation and care of your patients. I hope that I will be able to enhance your practice through prompt consultations and clear communication. I welcome your calls, and my office will do their utmost to accommodate your patients in a timely manner. Calls from physician offices will be given priority based upon professional opinion for either "same day" or "next day" booking. I look forward to the opportunity to participate in the care of your patients.

Dr. Walsh and his team would be honored to serve any of your patients who need a referral to a urology specialist. We do our best to offer same day appointments for physician referrals and promptly provide follow-up consult letters to referring physicians.

To schedule a new patient referral, please be prepared to give our office the following information:

  • Name
  • Social Security Number
  • Date of birth
  • Insurance carrier
  • Subscriber name
  • Insurance Contract number
  • Group number
  • Patient contact number

We also request the report from the patient’s most recent clinic visit from your office and copies of any lab reports.
